It had nothing to do with auto clickers, it depended on the ping (speed in witch your computer and server contact each other). If you had a great ping you had that few millisecond advantage to get the item.

First of all, autoclickers WILL get you banned, as there is an EXACT fixed time between each click, and is VERY detectable.
If you have alched with an autoclicker for 8hrs+ in a row and have NOT gotten banned for at LEAST Marcro (Minor), then you're EXTREMELY lucky.
Autoclickers will obviously lag the game, as the game cannot register the amount of clicks that are occurring.
Also, clicking an item to pick it up MORE THAN ONCE will get the action reset. This means if you click an item a million times to pick up, only the MILLIONTH click will register for you to pick up the item. This is why if you use an autoclicker to loot items, even without a delay on your clicks, the item will appear to be on the ground for nearly an extra THREE SECONDS before you actually pick it up.
